Please complete all steps prior to your visit. If you are scheduled for a Telehealth appointment, complete steps 1 – 4.

STEP 1:Create an Account. Use the following link to create your username and password, enter First and Last Name, Date of Birth and Zip code and agree to Terms and Conditions. Having trouble with link? Visit www.mypatientvisit.com and use the following Practice ID and Security code given in an email to you to create an account, check your junk mail:

Security Code: Given in an email sent from practice when scheduled
STEP 2:Complete Security Questions.
STEP 3:After login, select your name to take you take you to myDashboard. Go to the upper right corner, select myChart and complete:
- Insurance Information
- Upload Documents: Upload a picture of your Insurance Card, photo ID and any pertinent information needed for your appointment.
Please note it is necessary to complete ALL INFORMATION in every section. Incomplete information will not allow the system to successfully link to your Medical Record and we will have to reschedule your appointment until all data is completed.
Step 4: Download the MyPatientVisit App on your mobile phone. You will use this app to notify us when you arrive.
Due to the COVID19 pandemic YOU ARE REQUIRED TO WEAR A MASK. Patients without a mask will not be allowed in our office.
